Health and NHS journeys

Help getting to some health appointments when ordinary public transport is hard and NHS patient transport is not available. Not for emergencies.

This is not NHS Patient Transport, and not emergency transport

Community Ride Hub is not part of, and does not replace, NHS Patient Transport Services (PTS). If you have been assessed as eligible for NHS patient transport, keep using it. Community Ride Hub is never for emergencies — call 999, or 111 for urgent but non-emergency advice.

How this fits with Community Ride Hub Connect

Everyday trips such as the GP, dentist or pharmacy can often sit under Community Ride Hub Connect once you are a member. Membership contributions and journey charges have not been set.

This Health and NHS page is about journeys that need a partner or commissioned arrangement — for example recurring hospital treatment, discharge support, or pathways agreed with a GP network, hospital team or social-care partner. Any charge to the eligible passenger at the point of travel has not been set. The real cost is recovered from the partner, grant or contract that funds them.

What it is

Pre-booked, non-emergency community transport to and from health-related destinations, with support getting from home to the vehicle where needed — developed carefully with health and community partners.

Who it is for

  • People with a genuine mobility, health or disability barrier to reaching appointments
  • People who do not qualify for NHS patient transport but still cannot travel independently
  • People referred by a GP practice, hospital team, social prescriber or social care professional

How it would work

Journeys would usually be requested well in advance so a vehicle and driver can be planned. Many referrals will come through health or social care professionals. Members may also request some journeys directly once the right pathway and funding are in place.

Cost — for the passenger and for Community Ride Hub

Any charge to eligible passengers for approved health journeys has not been set. Community Ride Hub still intends to recover the operating cost through partner agreements, grants or the Community Mobility Fund — not by selling journeys commercially. See Funding and sustainability.

This is not a booking service yet

Community Ride Hub is preparing to operate. No health journeys can be arranged yet, and this page is not an offer of transport to any appointment.
