Refer someone to us

For GPs, nurses, social workers, occupational therapists, school SEN teams, day centres and other professionals supporting someone who cannot easily use public transport.

This is not a booking service yet

Community Ride Hub is preparing to operate. This page describes a proposed service. Journeys cannot yet be arranged, and nothing here is an offer of transport.

Please read this first

Community Ride Hub is not running passenger journeys yet. A referral helps us understand need and contact the person when membership opens. It does not book a ride today.

Who can refer

Any professional working with someone who may need community transport can refer them — with that person’s consent (or their legal representative’s consent).

Many people first hear about services like ours from a trusted professional. Thank you for taking the time.

What we need from you

  • Your name, role, organisation and contact details
  • The person’s name and how we can reach them
  • A short note on why transport is hard (keep it practical — no long medical history needed)
  • Mobility or access needs you already know about (wheelchair, companion, etc.)
  • Confirmation that the person knows you are referring them and has agreed

Consent — keep it simple

Before you send
Tell the person (or their representative) that you are contacting Community Ride Hub, what you are sharing, and why. Get their agreement.
On the form
You must tick that consent has been given. We will not accept a referral without it.
After you send
We will confirm we have received it. How quickly we reply is still [TBC].

What Community Ride Hub Connect offers (for you to explain)

Membership contributions and journey charges have not been set. Your organisation can cover membership and the journeys it agrees to through Referred Membership. Concession membership is for people on a low income. Its contribution has not been set either. Pre-booked, shared, door-to-door. Not TfL Dial-a-Ride. More on Community Ride Hub Connect.
